Service Description
$100 Deposit is required to hold the appointment. Deposit will be applied to balance of evaluation if the appointment is kept. For paid balances, deposit is only refundable with 24 hours notice of cancellation. If an appointment is cancelled without 24 hours notice then the deposit will be forfeited and will not be applied to any remaining balance.
